We are delighted to announce that Dr. Theresa Soriano, a nationally recognized leader in home and community-based care, has joined the Board of Directors for the Foundation for Social Connection Action Network!
She currently serves as the Chief Health Officer at Grouper, a company pioneering the social fitness movement, Dr. Soriano is deeply committed to fostering meaningful connections and improving health outcomes through shared experiences.
Dr. Soriano brings over 20 years of experience as a general internist, palliative medicine physician, and an advocate for addressing medical, behavioral, and social determinants of health. Her work spans academic institutions like Mount Sinai, innovative startups such as Cityblock and Prospero Health, and large health systems including Optum.
Her outstanding contributions to healthcare innovation have earned her numerous accolades, including the Hastings Center-Cunniff Dixon Physician Award and the AAHCM Physician of the Year award.
